Hospital Discharge Support Grant
Would you or someone you care for benefit from financial support to return home from hospital or ensure that you have the care that you need to keep well after a hospital stay?
Sirona Care & Health are offering a one-off discharge support grant of up to £1200 to help with a range of costs to help your recovery at home.
One-off payments of between £200 and £1,200 can be made to an individual, family member, friend or voluntary supporter to assist with your care needs.
